Lactation Support Access Sliding Scale Program

We believe every parent deserves access to skilled, evidence-based lactation support, regardless of financial circumstances. To help remove barriers to care, we offer a limited number of sliding scale appointments through our Lactation Support Access Sliding Scale Program.

**Please note: Lactation Support Access program appointments are available for in-office visits only. **

Who May Qualify?

• Participate in TRICARE, Medicaid, WIC, or SNAP

• Are uninsured for lactation services

• Are experiencing financial hardship

• Have unexpected medical, family, or employment expenses

• Need support that would otherwise be out of reach

No family should go without feeding support because of cost. We are committed to making lactation care accessible, supportive, and inclusive.

Confidentiality & Payment Policy

Confidentiality:
All information and documentation submitted for eligibility verification will be kept confidential and used solely to determine eligibility for Southern Lactation Services' Sliding Scale Program. Documentation will be securely stored and handled in accordance with applicable privacy standards.

Client Responsibility:
Clients must notify Southern Lactation Services of their intent to apply for Sliding Scale Services before scheduling an appointment and provide all required documentation in a timely manner. Eligibility cannot be determined until all requested documentation has been received and reviewed.

Payment Terms:
Payment for Sliding Scale Services is due at the time of service

Eligibility Verification:
Documentation is required to verify eligibility. Once approved, eligibility remains valid for six (6) months from the date of verification. Updated documentation may be required upon renewal.
